Our support is designed to meet children and young people where they are, and to grow with them as their needs change.

Outreach & Community Support

We build our support plans together with children and families, as well as with other professionals (if they are involved). We tailor our support to best meet the needs of the child and family. Seeking ways to provide the highest levels of support that can achieve the best outcomes.

Our support helps children and young people to:

  • Access short breaks and community activities
  • Develop everyday life skills, out and about in their own community
  • Build confidence, independence and social skills
  • Grow, develop and have fun along the way!

Amplify (Ages 14 to 18)

Amplify is a free group support and advocacy service for care-experienced and vulnerable young people aged 14 to 18.

Through Amplify, young people are supported to:

  • Develop confidence, self-esteem and identity
  • Learn practical life and independence skills
  • Build relationships with peers who share similar experiences
  • Access advocacy and trusted adult support
  • Have a voice in decisions that affect their lives

Continuity Beyond 18

Reach focuses on children and young people, but we recognise that turning 18 can be a significant transition. Young adults aged 18–25 continue to benefit from the full range of our support, advocacy, and guidance for as long as they need it.

This support helps to:

  • Maintain stability during change
  • Reduce anxiety and uncertainty
  • Support a smoother transition into adult services, education or employment

Initial meeting and assessment

We visit families to learn about medical needs, daily routines, and personal goals to capture the full picture of each child's life.

A tailored care plan

Our team designs a personalised plan that may include respite care, home support, and guidance for school and community activities.

Ongoing support

We hold regular check-ins with families, adjusting care plans as children develop new skills, face new challenges, or transition to new stages of life.
