Together, we
reach further.
Reach by Care Hub provides compassionate, nurse-led support for children and young people in Jersey.
We help families access safe short breaks, community activities, and tailored care that builds independence, wellbeing and belonging.
Reach provides flexible support for children and young people, with different options available depending on individual needs.

Our 1:1 or 2:1 support helps children and young people access short breaks and community activities, build friendships and develop independence.

Amplify
Free advocacy and group support for care-experienced and vulnerable young people aged 14–25, focused on wellbeing, identity, life skills and having a voice.
As children grow, their needs change. Reach supports children and young people at key stages, with care that develops alongside them.

Children (up to 12)
Support that feels safe, familiar and nurturing
Short breaks, respite and community-based support that help children feel secure and supported — while giving families time to rest and recharge.
Young People (12–18)
Building confidence, independence and identity
Support for young people to develop life skills, emotional wellbeing and social connections, preparing them for adulthood in a supported and dignified way.

Continuity beyond 18
Supporting safe transitions into adulthood
Where appropriate, familiar support can continue beyond 18 to provide stability, reduce disruption and enable a smooth transition into adult services.
These are the moments that matter most.
